The University provides a recommended format for the online PERFORMANCE APPRAISAL FORM. This form is designed for managers to log in and complete evaluations for all non-union staff who report to them. When evaluations are complete, staff will receive an email link that allows them to log in to their individual evaluation.
This form is new, and represents consolidation of the 5 forms we provided in the past, based upon feedback from surveys, focus groups and meetings conducted with our colleagues across all campuses.
Those schools and departments who have already done work on an alternative form are not required to use the new form. However, the new form will be only format provided by Columbia University Human Resources in fiscal year 2009 and beyond.
We have also developed a Performance Appraisal Job Aid to help you use the new form.
The other new feature for the online Performance Appraisal Form is the addition of performance attributes.
A performance attribute is defined as a skill, knowledge, ability or behavioral characteristic that is associated with job performance. Performance attributes assist both Managers and Staff in defining what job success looks like in terms of how the work is performed, not just in terms of what was achieved.

Writing goals can be a shared exercise between managers and staff. More experienced staff can and should contribute more to the process. We have developed a Staff Goals Worksheet that managers can share with staff to help write their own goals. Managers can then copy and paste the goals into the new web-based form.
