
HR has developed an easy-to-use Online Performance Appraisal Job Aid that will walk both managers and staff through the steps of how to use the new online performance appraisal form.
The new online performance appraisal form has a library of 54 performance attributes that managers can select from built into the form. As described above, these performance attributes can be used in various ways and at different levels at the choosing of the school/department or the manager.

For your convenience, we have developed sample Performance Appraisals for a fictional staff member in a fictional role at the University to illustrate usage of the new form for each of the four levels of performance: Does Not Meet Expectations, Needs Improvement, Meets Expectations, and Exceeds Expectations.
Writing goals can be a shared exercise between managers and staff. More experienced staff can and should contribute more to the process. We have developed a Staff Goals Worksheet that managers can share with staff to help write their own goals. Managers can then copy and paste the goals into the new web-based form.
