Back To TopDiscuss in advance the layoff with your local Human Resources representative as well as HR Client Services for approval. At the Medical Center, discuss the layoff with your HR Administrator. At Harlem Hospital Center, discuss the layoff with the Affiliation Human Resources Office.
Print an active PAF for the employee from the People @ Columbia system. Print/type information on the form, completing the required fields. Verify that the form is accurate and complete, sign it and route it to your local human resources representative for approval.
Prepare the appropriate layoff letter and/or release letter, as appropriate. Be sure to calculate the severance and enter the dollar amount on the PAF as well as the number of months or weeks.

Effective Date: Enter the date immediately following the employee’s last working day.
Term Vac Pay Dates: Enter the number of unused vacation days to be paid.
Severance Pay Amt: Your local Human Resources representative will enter the severance account number on the PAF.
Name, Title, Date: The names, titles, and dates of approval of you, any other approver in your department, and the local Human Resources representative.
