Any active Faculty and Staff enrolled in Oxford for 2007, who did not change their election during Open Enrollment, automatically became United Healthcare members effective January 1, 2008.
We would like to remind you that a UnitedHealthcare [Transition of Care application is no longer available as of January 31, 2008] (PDF) is available if your physician participated in the Oxford network but is not participating in the UHC network.
Transition of care is a service that UnitedHealthcare is providing to Columbia University employees so you can seek treatment from non-network physicians at the benefit level associated with UnitedHealthcare participating providers. Transition of care is available through December 31, 2008.
Please complete this form for each doctor providing treatment who is not in the UnitedHealthcare network and mail it to the address at the top of the form by January 31, 2008. Once the form is received and reviewed, your care will be covered for the 2008 calendar year only. After 2008, these providers will no longer be considered “in-network,” unless they formally choose to join the UnitedHealthcare Choice Plus Plan and are approved by UnitedHealthcare.
