Form
Effective in January 2001, all salaried employees who park in Columbia University or New York Presbyterian Hospital lots are required to have their parking payments deducted directly from their paychecks. Monthly billing and payment by mail was discontinued as of January 2001.
All other people having parking privileges receive monthly bills from the parking offices.
This form must be completed by each employee and returned directly to the parking office which manages their parking space. (The amount filled in on the form should be the amount you presently pay for parking as indicated by your parking office.) Failure to authorize payroll deduction of your parking fees will result in loss of parking privileges.
Please note the following:
NOTE: These new procedures apply ONLY to parking in Columbia or Presbyterian Hospital parking lots. If you participate in the transit portion of T/PRP or park in another parking lot, it is still necessary to enroll during Open Enrollment.
For further information about T/PRP, please contact Benefits, or read the T/PRP portion of your Benefits in Brief (see HR Documents for the latest version).
For further information about parking deductions, please contact one of the following, depending on where you park: for Institutional Real Estate, Karen Fry; for Morningside parking, Edrys Erisnor and for the Medical Center / Health Sciences, Wayne Patterson at (212) 305-1357 or wdp2@columbia.edu.
