University policies conform to the provisions of the Americans with Disabilities Act.
If you need a workplace accommodation, submit an Accommodation Request and a Disability Release to the Disability Services office. (These are available in the Forms box at right.) Please contact Disability Services if you have any questions.
If you have already submitted disability forms and would like to check on their status, please call (212) 851-0662.

The Return to Work Program facilitates the return of employees of the University who are out of work due to disability (non-occupational) or workers compensation. The program provides outreach and case management to all employees as well as referrals to various programs and services as needed.
Please remember to complete all applicable paperwork (e.g., the New York State Disability Form DB450) when going on a leave. For general information about leaves, Officers can consult Benefits in Brief; union-affiliated Support Staff should refer to the appropriate collective bargaining agreement.
If you have already submitted disability forms and would like to check on their status, please call (212) 851-0662.
